Lease Contents and Quotation
The customer may advise our service staff of your requirements, including car brand, model, time of usage, insurance coverage and maintenance needed. Based on your requirements, we will customize
your lease agreement and provide quotation accordingly.
Items that require further confirmation or discussion:
Car Brand and Type You will need to choose the car brand and type based on your needs.
Special Requirements Please specify if any special equipments are required (e.g., video-based driver assistance systems, GPS system or intelligent mobile phone), or if you need to display your company name on the car.
Lease Period Decide the most suitable period for the lease based on your actual requirements.
Insurance Coverage Apart from the third-party liability insurance, what is your required insurance coverage for the potential damage of the car? Do you require riders such as theft and burglary insurance?
Provides a Quotation
We will submit a lease quotation to the customer based on their requirements.
Signing of Lease Agreement
When the customer has agreed with the quotation, the Company will proceed with the document review and produce a formal agreement. Our service staff will continue with the relevant procedures including contract signing, guarantor verification and collection of guarantee deposits.

Long-Term Lease for Individuals (adults only)

Personal ID Card
Copy of Driver’s License (Both sides)

Employment-related Documents (e.g., Employee ID Card or name card)

Withholding Tax Certificate (Alternatively, payroll slip or copy of passbook including the credit transaction for salary)
Personal ID card copy of the guarantor.
Long-Term Lease for Corporate

Copy of the approval letter from the Ministry of Economic Affairs and the Change Registration Form.
Certificate of Ownership.
Personal ID of the responsible person.

Financial statement of the previous year (for tax-declaration purpose)
Form 401(Declaration of Sales and Business Tax by a Business Entity).

Personal ID card copy of the guarantor.
License Application and Delivery of the Car
After contract signing, Formosa Leasing Co., Ltd. will proceed with the car procurement and license application, etc. We will contact the customer to agree on the time and place for car delivery when all these procedures have been completed and deliver the car accordingly.
Commencement of the Long-term Car Lease
The customer will receive our long-term car lease service thereafter, and start to pay
the rental on a regular basis according to the agreement.
